We know that it can be difficult to know what documents are required for your reimbursement requests or where to obtain them. Below is some information about what documentation we can accept, possible ways to obtain those documents, and verbiage that you can use to help your provider understand what you need.
What documentation do I attach to my reimbursement requests for my insurance premium?
Your healthcare policy documents will need to confirm the following information:
- Type of insurance premium
- The premium effective date (for the current benefit year)
- The premium amount
Documentation that typically has this information:
- An insurance ID card that shows Part A/B, PPO, Medical, dental, or a document confirming the purchase of your COBRA, Medicare, dental, or vision policies.
- A welcome letter from your insurance provider
- A billing invoice
- Payment history that shows matching consecutive payments
- A combination of the documents above
What documentation do I attach to my reimbursement requests for medical expenses?
In order for us to verify your expenses, we need to be able to see the following information:
- The date of service or purchase date
- The services received, or item(s) purchased
- The cost to you
Documentation that typically has all of that information:
- Explanation of Benefits (EOB)
- Itemized Receipt (purchase)
- An invoice or Patient Ledger
We can only use credit card receipts to verify the cost of your services. They typically do not list the services, and payment dates can’t be accepted for the date of service. So you will need to provide additional documentation that shows the other requirements.
Insurance providers and doctor offices often have an online portal that you can easily access that typically has an explanation of benefits, visit summary, patient ledger, etc. If you need to reach out to your doctor or healthcare provider, you can explain what information is needed. They should be able to put that together for you. To save yourself a trip or waiting on the mail, they can email you an attachment or picture of the document.
